Application for Mileage Reimbursement

In order to be eligible to receive Mileage Reimbursement for the miles driven to and from your volunteer site, you need to complete this application form and mail it to Mesa County RSVP, Inc. before you can claim any reimbursement. Reimbursement is NOT retroactive and begins from the date your application is received.
